The PC-PTSD-5 Posttraumatic Stress Disorder Screening Inventory for Primary Care is a tool designed to identify individuals with possible posttraumatic stress disorder.
Sometimes people experience unusual or particularly frightening, traumatizing events.
For example:
- war;
- serious accident or fire;
- physical or sexual assault or violence;
- earthquake or flood;
- seeing someone killed or seriously injured;
- death of a loved one due to murder or suicide.
If you have ever experienced such events, please answer the following questions:

- Have you had nightmares about the traumatic event(s) or thought about the event(s) when you didn't want to?

- Have you tried not to think about the traumatic event(s) or tried to avoid situations that would remind you of the event(s)?
